I understand that a Certificate of Completion will be issued after the course of study is finished. The client must achieve a minimum grade of 80% in order to graduate. Clients will be permitted to retest for any grade below 80% and will be given a chance to be reevaluated. Permatech Makeup, Inc.'s Director may grant makeup time in the case of a death in the family (a copy of the death certificate will be required) or in the case of extreme illness (a doctor's statement will be required).
I understand that it is my responsibility to contact all of my State and Local Board of Health Agencies, regarding the Rules and Regulations for Permanent Cosmetics in my area.
I understand that my Certificate of completion is to aid in registration purposes and can differ from county to county and it is my responsibility to investigate prior to purchase.
I understand that hands on workshops and supervised live model work is whats needed to complete my education and is available to me at a cost.
I understand Permatech Makeup, Inc. cannot and does not promise or guarantee employment or level of income or wage rate to any client. The Enrollment Agreement is made according to the Agreement set forth which I have carefully read or have had read to me. I acknowledge that no verbal promises or statements contrary to the terms of this agreement have been made, and I certify that the aforementioned statements of Permatech Makeup, Inc. are true and correct. I understand this Agreement is legal and binding once it has been agreed to.
I understand it is my responsibility to keep a copy of all documents I have received from Permatech Makeup, Inc., including the Enrollment Agreement, Receipts, etc. In addition, it is my responsibility to maintain all records that prove that I have met my financial obligations to (PMI). I may request a copy of (PMI) records by submitting my request in writing at the Business Office.
I understand that all promotional material, client publications, information both verbal and written acquired by me from (PMI) is proprietary, and is not to be distributed or reproduced and is to be used for my personal understanding and information only.
I confirm that I am not acquiring information regarding any process or procedure of Permatech Makeup, Inc. for a competitor nor am I representing an entity other than myself.
I understand that after the product is complete is up to me to take what I have learned and use it as I see fit and that there is no guarantee that I will be a working permanent makeup artist. I understand that when I receive my certificate that the contract is complete. I understand that no refunds are given once the product has been purchased.
In signing this Disclosure Agreement, I state that all responses on all documents I have completed and signed are true and correct. I understand the Aforementioned Statements and Agree to abide by them.
I agree that any dispute between the undersigned Client and Permatech Makeup Inc.(PMI) aka Antoinette Cox arising out of or in connection with any procedure performed by (PMI) shall first be submitted to mediation before Judicial Arbitration and Mediation Services (JAMS) in Ventura County, California, with each party sharing the costs of such mediation equally, payable to JAMS in advance of scheduling any such mediation session. I agree that if such mediation fails to resolve any such dispute, then the dispute shall be submitted to arbitration before JAMS. I agree any award shall be final and binding upon the parties and judgment of such award may be entered in any court or tribunal having jurisdiction. The prevailing party at such arbitration shall recover reasonable attorneys' fees as an element of damages.
